DHFS IMPACT Provider Enrollment Services has opened a temporary revalidation window to allow groups to submit the necessary revalidation application.

Please note that revalidation must be completed before December 31, 2026. Failure to do so will result in claim denials. Once IMPACT staff have reviewed and approved the revalidation, your group's enrollment status will be reactivated.

To assist you in completing this process within the required timeframe, we are including the following resources:
